Routine exercise testing could not predict T‐wave oversensing in a patient after a subcutaneous implantable cardioverter‐defibrillator implant

چکیده
Subcutaneous implantable cardioverter-defibrillators (S-ICDs) are susceptible to T-wave oversensing (TWOS) caused by high rate-dependent QRS-T morphology changes. We experienced an inappropriate S-ICD shock due to TWOS, which could not be predicted by routine exercise testing. A newly available high-pass filter might be effective for avoiding this.

Importance of exercise testing shortly after subcutaneous implantable cardioverter-defibrillator implantation in patients with Brugada syndrome – The first case of associated inappropriate shock in Japan
We report the case of a 51-year-old patient with Brugada syndrome (BrS) who experienced inappropriate shock due to T-wave oversensing (TWOS) during exercise when the optimal sensing vector was selected based on the automatic analysis by a subcutaneous implantable cardioverter-defibrillator (S-ICD). After selecting another vector during exercise testing, TWOS did not re-occur. Selection of appro...
متن کاملA case of inappropriate implantable cardioverter defibrillator therapy induced by T-wave oversensing due to hyperkalemia.
There have been reports of hyperkalemia-induced T-wave oversensing in patients with implantable cardioverter defibrillators (ICDs). However, a comparison of T-wave amplitudes and morphologies between the surface 12-lead electrocardiogram (ECG) and ICD electrogram has not been reported. We present the case of a 70-year-old man who received inappropriate ICD shocks due to hyperkalemia-induced T-w...
متن کاملInappropriate shocks from a subcutaneous implantable cardioverter-defibrillator due to oversensing during periods of rate-related bundle branch block
The subcutaneous implantable cardioverter-defibrillator (S-ICD) is a novel technology with proven efficacy in sudden cardiac death prevention; however, there is a lack of long-term safety data. We describe the case of a 55-year-old female patient implanted with an S-ICD due to idiopathic ventricular fibrillation, who subsequently presented with inappropriate shocks leading to ventricular fibril...
متن کاملSubcutaneous Implantable Cardioverter Defibrillator: Early Experience
INTRODUCTION The implantable cardioverter defibrillator had been increasing the survival of patients at high risk for sudden cardiac death. The subcutaneous implantable cardioverter defibrillator was developed to mitigate the complications inherent to lead placement into cardiovascular system. OBJECTIVE To report the initial experience of 18 consecutive cases of subcutaneous implantable cardi...
متن کاملSubcutaneous Implantable Cardioverter Defibrillator in Patients With Hypertrophic Cardiomyopathy: An Initial Experience.
BACKGROUND The subcutaneous implantable cardioverter defibrillator (S-ICD) has been developed to avert risks associated with transvenous defibrillator leads. The technology is attractive for younger patients, such as those with hypertrophic cardiomyopathy (HCM). However, there are limited data on S-ICD use in HCM. METHODS AND RESULTS HCM patients identified at risk for sudden death were consi...
